This session is designed to bring together an enthusiastic and multiskilled team from across the ICS to generate ideas and solve key issues for patients in the Cheshire and Merseyside ICS.

Our core objectives for the hackathon are to:

• Accelerate population health management across the ICS
• Support development of integrated care solutions
• Empower the teams with enough data to get to the heart of the problems using System P analytics resources
• Translate these insights and apply them practically to challenges facing the ICS

The session will focus on Frailty and Dementia and Complex Lives as two priority groups with the greatest potential to develop workable solutions at both place and Cheshire and Merseyside system-level. The session will be an opportunity for places to explore these two priority segments in greater depth.
